Healthcare Declaration

A Healthcare Declaration consists of two parts (1) a Healthcare Directive and (2) a Healthcare Power of Attorney. 

A Healthcare Directive is a written document used to convey your healthcare wishes when you aren’t able to personally communicate those wishes due to various physical or mental limitations. 

A Healthcare Power of Attorney is a written document where you simply appoint an individual to represent you, overseeing and executing your specific wishes and preferences regarding healthcare by providers such as doctors, nurses, etc. 

Both of these documents become effective upon incapacity - when you are no longer able to make your decisions. It is extremely imporant to have these documents in place while you have the ability to makes these decisions. Otherwise, your loved ones may have to ask the Court to appoint someone to help manage your affairs should you become unable to do so.
